Pregnancy unlikely: If 18 days after a missed period, your HCG is less than 5, you are unlikely to be pregnant. Menstrual irregularities are not uncommon and anovulatory cycle is a common cause. HCG: Increased HCG levels do not automatically mean pregnancy. Some tumors might secrete hcg, too. We would need to know more information -age, history of intercourse (s), etc.

: Conception occurs within day or so of the sex. It takes about 3 weeks for the embryo to make enough HCG for a positive pregnancy test. Blood test may be done a day after a missed period, HCG more than 25 indicates pregnancy. Home pregnancy tests should be done 4-5 days after the missed period. If the result is negative repeat the test in 4-5 days. Use first morning urine.

Implantation must : occur first, before HCG is measurable.  7 or more days is usually required, successful implantation can occur. However, in life, strange things are remotely possible. An ultrasound measurement of the fetus when 7-8 weeks along can be very accurate in picking due date and counting back to conception time.

: You were pregnant from a previous encounter. Six days is not sufficient for the HCG level you described. It takes about 3 weeks from sex for the embryo to make enough HCG to reach the level you described.

Yes: Although the pregnancy is more likely due to a different episode of intercourse than the one the day before taking the test.  Alternatively, considerably much less likely, you could have another source of HCG produced in response to another (non-pregnancy) medical condition.
