Any: physical illness, particularly requiring hospitalization can affect your menstrual cycle (possibly related to hormones released by stress)  Things should return to normal in a few cycles....You should get your IRON checked (particularly if you had a lot of blood tests!) Iron deficiency can affect your menstrual flow.

DUB: Dysfunctional Uterine Bleeding can be caused by many things.  You need to see the GYN doctor to determine your cause.  Best wishes.

Absent period/stress: By your own admission the absence of your period in spite of negative pregnancy tests is driving you "crazy". That type of  "crazy" impacts the pituitary gland and has been known to absolutely STOP periods. An example, a death in the family with period absence. If no period and negative pregnancy tests in three months, see a GYN MD who can  hormonally "jump start" your period.

True: Very unlikely you are pregnant, however, may do a pregnancy test for peace of mind. Periods can be irregular on the implant or you may start to skip them as well

Take test : If you think you might be pregnant check then check a home pregnancy test. They are cheap, reliable and available  at any pharmacy. Use over the counter Plan B if inter course  was within 72 hours to decrease the risk of unplanned pregnancy.  Start birth control to avoid these scares

HIGHLY unlikely: Nexplanton is the most effective contraceptive available. It's as good, or better, than sterilization. In 11 clinical trials in which 942 women enrolled for two to four years showed the etonogestrel implant was well-tolerated and effective: no pregnancies occurred(UpToDate). You : You should start with seeing a family practice or internal medicine physician.  They can perform an evaluation and refer you if you should need a specialist.  If you can't find a doctor to see you right away, or if you have continued shortness of breath, cough, fever, or new symptoms, then you will have to go to the er.

ultrasound showed iud in place and normal scan, but have had periods for 4 months after no menses in the 2 years on mirena. could it not be working?

Unlikely : But never say never.  Use condoms until you check with an OB/GYN.  I'm not a big fan of IUDs,; anecdotally, I've just seen so many women with complications both shortly after placement, months or years later, and problems with menstrual cycles, miscarriages and trouble getting pregnant for years after having it removed.
