my 7 week infant received his first vaccine shot in his left arm, is this a cause for concern? it has now suddenly swelled up and has some form of abscess surrounded by redness. should i see a doctor?

Maybe: If the baby is otherwise eating and doing okay. Swelling of the site of vaccination is common. You do not need to worry just continue good hygiene and use warm compress for the area and try not to mess with it because it is also painful. If the baby is having other symptoms like not eating or high fever or anything else concerning then let a doctor recheck the baby.

Hard : Hard to tell sight unseen and without a radiograph, but it sounds like the abcess and broken tooth you left untreated for such a long time has created a substancial amount of bone loss around that tooth. It could be that the root end is poking through the hole in your bone and sticking into your gum tissue.

Not a healthy situation.

Obviously you understand that you need to go see a dentist to have the tooth extracted. The dentist who extracts the tooth will determine if it is indeed the root end or something else. Please go and be seen as soon as possible to avoid further complications or massive infection.

Puss under pressure.: Abcesses are puss under pressure.  They get very hard when there is no way for the abcess contents to drain.  Generally they will try to break through to an adjacent structure or through the skin to relieve the pressure.  If this is not possible they will become rock hard and very painful.  Eventually all abcesses rupture and bring great relief, but a scalpel is sometimes helpful.

Decay: A deciduous tooth, like a secondary or permanent tooth can have an abscess if bacteria get into the pulp tissue. Bacteria decay the tooth, and if untreated, it can abscess. A deciduous tooth can be saved by having a root canal treatment, if a second tooth is not coming. Ask your dentist for a referral to an endodontist if they don't do root canal treatment on a primary tooth.
