Sounds unusual: If this is the first time it has occurred it is most likely an allergic reaction of some type and shoul resolve with a topical hydrocortisone. If it worsens, recurs or does not improve, i would seek the care and evaluation of a dermatologist.

Pompholyx?: Dermatitis either the atopic (e.g. pompholyx eczema) or irritant form is quite possible here.  Other possibilities include a variety of inflammatory joint disorders and even some forms of autoimmune diseases such as vasculitis.  Variety of possibilities therefore.  Best way to sort through them and sort out diagnosis and treatment would be a consult and exam with doctor and/or run tests.  Good luc

: Swelling and itching of hands warrant an urgent visit to a dermatologist, an Urgent Care facility, one's physician, or a hospital Emergency Department.  The task is to determine the underlying cause of these symptoms.  Causes include contact substances inciting an allergic skin response, a systemic allergy to some ingested substance, a food allergy, a systemic disease, or some other cause.

Cholestasis: Talk to your doctor about checking your serum bile acid levels. To stay in low risk levels they need to be less than 40. There are treatments. If levels are high there may be some increased risk. Talk to your doctor.

Dyshidrotic Eczema: Dyshidrotic eczema is a condition in which small, usually itchy blisters develop on the hands and feet. People are more likely to develop dyshidrotic eczema when they are under stress, have allergies, their hands are often in water or are moist. The cause is unknown.  Treatment is usually with strong topical prescription cortisone like clobex, vanos, or ultravate.

Possibly: It could even have been eczema from the beginning; sometimes they're hard to tell apart. Make sure you're using hypoallergenic skin cleansers and lotions, and if you've already used a fungal cream 3 times daily for 2 weeks without improvement, stop that and start Hydrocortisone 1% ointment 2 times daily. If not improving after a few days, please see your doctor soon.

Without : Without examining you and being able to see the rash first hand, it's difficult to say what it is and how you should treat it.

-if it is a contact dermatitis caused by an irritative substance such as poison ivy, you want to wash the area well with soap and warm water and apply hydrocortizone for the itch.  If it is spreading, worsening, or not improving after a week, you should contact your doctor for an exam and possibly stronger prescription medication.

-if it is ringworm (a fungus causing an itchy, raised circular ring), you can try using Clotrimazole cream 2 or 3 times a day for the next few weeks.  It may take 3-4 weeks to completely disappear and is contagious for the first few days you are being treated, so wear a bandage over it.

-it could also be a chronic skin condition such as eczema or psoriasis, just to name a few, which may require stronger prescription medications for treatment.

If it is not improving or if it is getting worse, the best thing to do is to call your doctor for advice.  They may want to take a look at the rash in order to diagnose and treat it properly.

There : There are several possibilities including psoriasis, eczema, lichen planus, and skin allergy.  I would recommend that you only use a gentle cleanser when cleaning your hands (like cerave or cetaphil), and use a moisturizer cream such as aquaphor, cetaphil, eucerin, or cetaphil cream twice daily.  If not better after a week or so, consider a trip to a dermatologist who can make an accurate diagnosis and formulate a good treatment plan for you.  You might need a steroid cream/ointment to clear this up. I hope that helps!

Sounds normal: Healing stitches can itch like crazy. ( I have personal experience with this). You might want to peek under the bandage and make sure you haven't developed a rash from the bacitracin or something else.

Contact allergy?: This could represent dyshidrotic eczema, a contact allergy to things you are touching, or even a localized version of psoriaisis. Over the counter products will help a little but if this is persistent you need to see dermatologist for treatment and possible allergy testing.
