for over 2 years the top of both my shoulders really hurt to touch and when i turn my head left all right and back and forth i can here like a crackling and grinding noise what could it be ?

Degeneration: You are describing crepitation in your neck, which most likely is caused by degeneration (normal aging changes) in your cervical spine, with associated inflammation.  This is very common and nothing to worry about.  However, if your symptoms worsen or become more concerning, do see a physician.  The shoulder pain is likely regional radiating pain from your cervical spine.

Stretching back: Sometimes the lack of stretching of back and spine can produce these pop sounds while doing sit-ups. Check with a physical therapist or a trainer to make sure you are doing the exercise properly without putting stress on your back.

Starting : Starting to exercise is hard on the body especially when you were inactive.  It sounds like the "insanity" workout is too stressful on your back.  You would probably benefit from stopping "insanity" and then start with a beginner workout and slowly build up to "insanity".  Last thing you want is to hurt your back.  I would discuss your pain with your primary care doctor, physical therapist and trainer.  Good luck!

Instability : The collar bone articulates with the sternum medically and the acromion laterally.  It is held in place by ligaments.  It sounds like at one of these joints, there must be some instability present, which can occur for a number of reasons.  I would not hesitate to be seen and thoroughly evaluated to discuss treatment options.

WearBack Support: I am sure your back surgeon and physical therapist have given you guidance. Please follow those. Do not ride in the car for long distance, avoid high impact exercise like running/biking. Try stationary bike/elliptical instead. Sitting for long times also hurts. Gentle stretching helps.

Get checked : You may have torn your meniscus or you have advanced cartilage wear down. You need to be seen by and orthopedic surgeon, get x-rays weight bearing, and then an MRI to evuate the cartilage/meniscus.
