Try CBT: All research comsistently finds that optimal treatment includes good CBT therapy and your active involvement. With or without medication, results are sugnificantly better. You learn skills to manage your life overall and if you miss this part - it's very hard to make things better. Best wishes!

hi,  i have beem taking 60mg prozac for severe deppression, but has stopped working after 8 years, the doctor tried remeron on top of it but t hasnt made any change , now he is stopping prozac and starting me on zoloft and still keeping me on mirtazapine

CBT ever tried?: There are a plethora of medications available for the treatment of depression. But a question- Are you being treated by a psychiatrist for this or is it your family doctor managing all these changes and strategies?  If the former then, fine, if the latter then, don't be shy to ask for another opinion. Next, Cognitive Behavioral Therapy has raised some brows lately. Could it be a good change 4 U?

Remeron (mirtazapine): Remeron (mirtazapine) has a relatively low incidence of sexual dysfunction. I have not used remeron for this purpose but have read about colleagues using it off-label for this purpose.

Therapy?: Have you yet tried any form of psychotherapy? Typically, the optimal combination of things to treat emotional issues such as anxiety or depression is medication (which, on your best day, is supposed to take the edge off symptoms) but the real gruntwork takes place in psychotherapist's consulting room.

Discuss w/ provider: Hopefully the same doctor prescribed these or they consulted if it was not. There is a potential problem (serotonin syndrome) when Zoloft (sertraline) and Remeron are taken in combo. So, make sure this is the best treatment plan available to you since it needs close monitoring. They all interact w/ each other in a non-major way that can increase dizzy, drowsy, confused effects so be careful w/ what you do.

Talk to your doctor: and/or your pharmacist.  Presumably the same doctor is prescribing all your medications. If not, they all need to know what you are being prescribed so that you can safely take what they are recommending for you. Remeron (mirtazapine) is commonly used on the meds you are taking, but without knowing your full history, other meds etc., I cannot tell you it's safe or not. Talk to your prescriber(s) &amp; pharmacist.

Depends: This would be a decision you would make in partnership with your physician. Most antidepressants are thought to be = effective. Zoloft (sertraline) is an ssri and Remeron also blocks serotonin reputake, but in addition block Alpha 2 adrenergic receptors. It tends to be more sedating, more likely to cause appetite increase, less likely to cause nausea, and less likely to cause sexual side effects than zoloft (sertraline).

Not the first: Although more commonly seen with remeron, zoloft and any ssri antidepressant can cause weight gain. In general, a 10 to 15lb weight gain is reported by some, but not all, with remeron. Weight gain with zoloft is usually not as dramatic, but there are exceptions of course. Of the two, zoloft is less likely to cause weight gain. Wellbutrin and Effexor (venlafaxine) are least likely to cause weight gain.

Switching Antidepres: Cross titration is recommended. This is done under supervision by your doctor to avoid withdrawal or recurrence of symptoms. Gradually tapering one medication while introducing &amp; gradually upgrading the other is how it's done.

Look for underlying: I assume SAD refers to social anxiety disorder vs seasonal affective disorder vs schizoaffective disorder

Finding the underlying cause is always better than taking a pill to mask the symptoms and create more symptoms.  Looking at neurotransmitters.  For example pyrrole disorder (which can be helped by zinc and vitamin B 6 ) can explain the  social anxiety symptoms.  Try desensitization, relaxatio
