Weight loss: There is no one plan fits all. In general  eat less and exercise more. Many structured plans can work ie weight watchers, 17 day diet, low carb. All have been successful for many.

The olympic runners would all be "underweight" while ultra-fit football linebackers are "obese". If your body looks and feels right to you & you compete successfully in sports, you're at a good weight.

Ok..: Get your stool examined for Ova and parasite. Get you CMP and TSH checked. Change your diet to more protein and more calories. Depending on your physical activity the calorie is assigned. Take more calorie dense food...you will gain weight easy.

See Dr., then eat...: If one is truly underweight (based on height, family background, and body type), a dr. Can evaluate for a hormonal, digestive, or other disorder. If no medical cause is found, then one can add weight by eating more calories of nutritious foods, while also doing weight-training to increase muscle. Proteins: egg white, chicken breast, salmon, ...  Better fats: olive oil, peanuts, almonds, tofu, ....

No: His body mass index would place him in the slightly overweight range, and in and of itself he might otherwise be healthy. The heavy smoking however is a very great health hazard and places him at higher risk for heart disease, cancer, emphysema, stroke, etc. If he a smoker does one thing for their health it would be to quit.

Depends: It depends what you mean by a good supplement.  Most weight training supplements are considered by the medical profession as a waste of money at best, and potentially dangerous at worst.  One needs to understand what one is taking, and what the risks, benefits and alternatives are.

See Dr.: If you are 5 feet 10.5 inches and only weigh 65 pounds? Then you are severely underweight.  See a dr. For an evaluation.
