Hamstring Strain: The standard principles of rest, ice, compression, and elevation apply.  Modify your activity accordingly. Tears of the muscle, whether partial or complete, do not respond to surgical intervention and are treated conservatively. Full thickness tears of the tendon usually require surgery, so get evaluated as soon as possible. An MRI may be necessary to determine the extent of your injury.

Mostly yes: Muscle tears generally heal due to good blood supply. If you have further concerns, discuss with your Ortho surgeon who saw this area at surgery.

Hip impingement is: a condition in which extra bone grows along one or both of the bones that form the hip joint, causing the joint space to be compromised.  This is often caused by mild trauma from overuse or repetitive flexion at the hip.  Tendinopathy is inflammation of the tendon that attaches a muscle to the bone and can be from overuse, acute trauma, or impingement.  It is not a tear of the muscle itself.

Physiatrist: I would recommend seeing a physiatrist. These are doctors of physical therapy. I would also consult your surgeon and possibly get a second opinion.

Tear.: Sounds like the pain your having is caused by the gluteus tear.  Hopefully, this can be healed without surgery, but you might need an orthopedic surgeon to evaluate it.

Not my regimen!: I would inject your tb, and then put you on a cycling regimen of graduated increasing intensity.  I do not know what pt has to offer bursitis except great medical expense! bicycling is the best sporting activity for gluteal issues, because it does not overload, but increases blood flow immensely, which helps heal sprained/strained muscle.

is prp (platelet rich plasma therapy) effective for severe tendon tears (gluteus minimus and medius)?

PRP unproven: Prp has been used for tendonitis in the elbow but has not been studied in hip muscles.  It will not aid in the repair of complete tendon tears, but may aid in the resolution of tendonitis around the hip.  If you have a complete tendon tear, you need surgery to repair the ruptured tendons.  It is much more likely to have tendonitis which may respond to prp.  I would try pt and nsaids before prp.
