The sonographer will put ultrasound gel on the skin for seamless transition of the ultrasound beam from the transducer(scanner) into the body.  The sonographer will glide the scanner over the skin, and images will appear on the screen.

There is a solution: You need a good internist to guide you with the perineal hygiene and monitor the problem with urine culture and use of appropriate antibiotics based on the urine culture results. Long term antibiotic prophylaxis is sometimes the  best solution if all else does not succeed.
