Bursitis. see a doc: you have what we call bursitis.

you could see your doctor ,for aspiration and treatment they usually keep coming back.
you might see a surgeon for possible surgery to have it removed.
it is an inflammation caused the fluid to leak outside the joint ,anti inflammatory medication could slow down the process.

Unlikely: Usually ganglion pain is localized to the affected wrist. However if it is affecting movement and one is compensating for the pain it could cause referred pain to other joints on the same extremity.

Wrist pain: Maybe the pain is not form the ganglion issue. Follow up with the wrist surgeon to get another eval since you are still having pain 4 months post-op.

No : No, I would not be worried, but if you are I would not hesitate to be seen.  An inflammatory condition, such as lateral epicondylitis, is among the most common causes.

Specific: Is the mass on the arm between the shoulder and elbow? Is it getting bigger  and over how long a time? Does it pulsate? Does it move with the shoulder or elbow? Does it "tingle" when you thump it? These are things to think about when you see you see an orthopedic surgeon. He can decide what to do for you.

If it bothers you: Ganglion cysts are benign; if they aren't bothering you, best to just leave it alone. If it's bothering but you don't want surgery, your dr may be able to inject it with cortisone, which should get it to shrink & quit bothering you. If that doesn't work, an orthopedic surgeon can remove it.

See a breast surgeon: Your wife should undergo full workup of the cyst and she may consider removing it surgically to help with the pain and to confirm that it is in fact ganglion cyst.

Need evaluation : You need to see your PCP ,looks like it can be carpal tunnel syndrome or can be some other disorder ,but you need further evaluation
