See below: Penicillin is excreted in the urine.  It causes a distinct medicinal smell that i would not describe as foul.  It smells the same way the penicillin tastes in tablet form or how it smells if you melt a tablet in water.

Yes: Foul smelling urine can indicate a possible urinary tract infection.  You need a urinalysis and urine culture done by your physician to assess and possibly treat if you have a uti.  Also, the underlying risk factors for a UTI must be assessed for men and women both, especially if recurrent utis occur.

Symptoms of UTI: The symptoms are frequency of urination, frequently burning associated with urination and often a cloudy urine.  A foul smell of your urine can be a result of food eaten such as asparagus which due to one of it's components leads to foul urine..

Diverticulum?: There are several possibilities, one of which would be a bladder diverticulum. You do not say whether it is possible that the odor is coming from vaginal secretions, or whether you have been seen by a urologist and undergone cystoscopy.  If not, do so.

test to see what the bug is that is infecting your urinary tract AND what antibiotics will kill it. Once you know that your doc will prescribe an effective antibiotic for you.
