You would need to : Be evaluated, so many things can cause swelling. The fact it is unilateral adds to the differential diagnosis a blood clot. Usually, in a blood clot the calf would be swollen or painful. (Though, doesn't always have to be)

i inject drugs and missed a vein on top of my foot. it hurts really bad and my whole foot and ankle are swollen. it also blistered. what can i do?

Get Help STAT: You could have any number of problems, but my guess would be either a possibly severe infection or a blood clot.  I recommend you go to an emergency center stat as if an infection, this usually requires intravenous antibiotics to fix and if a blood clot in your leg, you really cannot treat that at home either.  You have another problem that needs help obviously as well.

Ankle pain: Ankle pain is a complex animal....And, before you can heal the pain, you have to know the cause... For example, chronic or acute synovitis, impingement lesion, joint effusion, chronic strained ligaments...Just to name a few. 1st, find out what is wrong, and then ask your choices..  
See a specialist, get examined..X-ray, mri..Whatever is needed, and then you will know.

Unlikely: I think that it's very unlikely that an injury/incident from 10yrs ago would lay dormant then come back to haunt you now but stranger things have been known to happen. With that said, your description of your complaints is too vague for a definitive answer, which is best addressed by your family doc who can examine &amp; order appropriate tests to arrive at diagnosis and offer you correct treatment.

Yes: Azathioprine is a reasonable choice after prednisone, especially if the Prednisone was very optimally effective or the skin lesions could not be adequately controlled on a low dose of prednisone.

Ibufrofen, an NSAID,: Is an anti-inflammatory medication.  Its nature is to calm down inflammation.  Ankle sprains need ice and antiinflammatory medication such as Ibuprofen in the 1st 24-48 hours to combat swelling and inflammation, then heat afterwards to improve mobility.  Add on compression in the way of ankle support and that is the basis for treatment for mild to medium sprains.

Ankle pain: Ankle sprains often do not heal quickly because people walk on the ankle. Much more information is needed to answer this.  What is important is to understand is why you have a "high temp" -is it a fever (temp 100.4 or greater)?  Is it possible infection or fracture not seen on the original x rays?  Look up Ottawa rules

RICE: Ice, elevate and rest.  Cavemen didn't have antiinflammatories either and this is the tried and true remedy for most soft tissue ailments.

don't you normally get drugs to set a broken bone (lets say ankle) prior to a doctor putting on a cast?

Depends: Not all broken bones need to be "set". Some are non-displaced. In that case they don't.
