Scope and biopsy: During upper GI endoscopy and biopsy of the suspicious lesions. Also barium swallow, ct scans , pet scans , ultrasound endoscopy all add to the diagnosis.

Q: my grandad has just been diagnosed with esophagus cancer & 2 blood clots on his lungs what treatment will he get as apparently wharf in can't be used?

Injectable: Lovenox (enoxaparin) is the drug of choice if warfarin cannot be used

Not at this time: If there is not family history of colon cancer you can delay a screening colonoscopy to 50 years of age. You may search FODMAP to lean about gas causing foods. Q: will alkaline water and foods help prolong the life of a liver cancer patient stage 4? what could be done to the patient without any further treatment

No change: Alkaline water is no better different than bicarbonate (sodium bicarbonate) added to your water but it is expensive. I am sorry to hear about stage 4 liver cancer. Talk to your doctor about any research for patients with your type of cancer and if the nih is offering trials for nutrition in cancer. Talk with family members about goals and expectations in an open manner and expect more than one discussion.

Which cancer?: Adjuvant (post surgery) treatment does have an end. Some metastatic treatments usually end at a maximum number of cycles of therapy (eg lung cancer - 6). Some treatments are continued in the advanced/metastatic setting until progressive disease or toxicity because they have been proven to (or thought to) improve survival.

Genetically differen: Cancer is a disease caused by bad genes but everyone's genes are different. So there are no two individuals whose cancers are entirely alike even with the same/similar diagnosis.

Esophageal fistula ?: Most likely she the tumor that is eroding into  and caused fistula, indicative of advanced cancer, non curable at this stage, various palliative measures are possible to make her comfortable at this endstage , like placing a silicone tube  in side , or feeding with  gastrostmy tube.And taking help from hospice care and make her life as comfortable as possible.

Multiple : Major side effects of surgery- healing problems, eating problems (small frequent portions, dumping syndrome), pain control. For chemotherapy- commonly nausea/vomitting, low blood counts, weight loss, diarrhea, mouth sores, infections. For radiotherapy- commonly fatigue, heartburn, skin reactions, esophageal sores, low blood counts.

Probably surgery.: Barretts esophagus is a precursor to adenocarcinoma of the esophagus.  Treatment is chiefly with surgery.  Curative resection is rarely possible, with <20% 5-year survival in patients with resectable tumors.  Palliative measures include laser ablation, mechanical dilatation, radiotherapy, and luminal (phenobarbital) prosthesis to bypass the tumor.  Gastrostomy and jejunostomy are done for nutritional support.
