: It is an abnormal finding, especially at your age; however, it less likely affects your total kidney function.
I wonder if you had a serious kidney infection or your right kidney was injured (motor vehicle accident, sport injury ..) in the past.

Normal ultrasound: It means your ultrasound of the right kidney shows obstruction of the urinary tract &amp;#40;that is hydronephrosis&amp;#41;, no kidney stone &amp;#40;no nephrolithiasis&amp;#41;, and no focal mass/tumor. Better be evaluated with CT urogram/ pyelogram and a urologist for the site and nature of the obstruction!

i have focal scarring on upper and lower poles of the left and right kidneys. also i have a simple cyst that measures at 4.7 cm on my left kidney. should i be concerned about the scarring or the cyst?

: If your kidney function is normal there is nothing to worry about. Just have periodic followups with the Urologist to keep an eye on both. Good luck

Chronic Pyelo: Chronic pyelonephritis is the result of any process in the kidney that causes prolonged inflammation and fibrosis (scarring). Common causes include: persistent or recurrent renal infections, vesicoureteral reflux (urine going backwards into the kidney), or urinary tract obstruction. A board certified urologist or nephrologist are the best doctors to review these types of findings.

Not to worry: This is rare and i trust the diagnosis is correct (i.e., not a scarred kidney from illness) and there's no genetic syndrome obvious. I'm also assuming that this was discovered incidentally, that your left kidney is normal, and that your kidney function is good. You can't donate your left kidney to somebody you love. If i'm correct, you just need to protect your good kidney.

Urology evaluation: Further evaluation depends on the symptoms and abnormalities in labs found if any. If this was found with an US it might need a CT scan follow up. You need to discuss with your doctor and consider Urology evaluation for follow up on this.

Depends: If it is simple cyst nothing. Most cyst are benign and people develop then with age without any harmful effects. If the radiologist read this as complex then it would need to be evaluated further with more imaging.
