There is a lot: Of variation from patient to patient.  It is important to get the surface of the bone (periosteum) well numbed.  There is a sensation from "tug" to "significant brief discomfort" when the liquid is aspirated from the marrow space, but it only lasts seconds.

Many diseases can do: Radiation and chemotherapy are common causes. Cancer can get into the bone marrow and replace it, thus crowding the bone marrow cells and prevent normal function. Sometimes the cause is unknown..It is called aplastic bone marrow( aplastic anemia). Cancer of the blood cells(leukemia)can also suppress normal bone marrow and cause low blood counts or sometime high blood counts(but few normal cells).

Many reasons: Marrow can be hypercellular because any of the cell lines, red cells , white cells or platelets or intercellular matrix  may start to grow due to benign or malignant causes. Bottom line is we have to look for clones or abnormal cells and/ or poor Cytogenetics or molecular markers

I can't think of an exception to bone marrow involvement in leukemia (although maybe some case report does exist...).

Side effects: The donor for a transplant typically gives peripheral blood stem cells rather than bone marrow.  This involves getting some shots under the skin for a few days and then cells are collected from the blood.  There usually is not much discomfort.  
Bone marrow harvests require anesthesia so the donor does not feel discomfort during the collections but may have mild-moderate sitffness or soreness.
