Allergy: it could be an allergic reaction in spite of the black coloration the itching could be the best sign 
Take Benadryl (diphenhydramine) and see a doctor

Probably keratosis : This sounds as if these are seborrheic keratosis that can either be frozen or scraped off. Some docs may also use lasers for this.

Chronic urticaria: You have hives. Hives lasting more than 2 months are characterized as chronic. About 95% of chronic hives are a benign but uncomfortable manifestation of autoimmune disease. The other 5% may represent a more serious condition. A board-certified allergist can perform a few tests to tell you what is going on. The allergist will work with you to provide adequate treatment. An antihistamine may help.

Rash: Herpetic Whitlow is a painful cluster of the small vesicles on the tip of the finger. Another possible cause of the similar rash is an  impetigo, or bacterial infection.
 In the case of dyshidrosis, there could be small vesicles under the skin of palms and fingers. It cause itching and the skin may became scaly. Dyshidrosis could be treated with the steroid creams.

Rash: Not likely a breast cancer, more likely a skin disease like eczema, which especially with pendulous breasts may become irritated or infected. First see your pcp or obg for a good breast exam including palpation for masses or nipple discharge. This may just be a skin disease, but should be checked.

Cause of itch?: Whatever the cause of the itching, repeated scratching will cause skin spots and lesions as a result. Sometimes moisturizing cream can help. Or calomine lotion if the itching continues. See your doctor if not better in a week.

Need info: If your scrotum has itchy white bumps under the skin you may have inclusion cysts, infected hair follicles, etc.  If the spots are on the skin and flaky you may have a fungal infection.  Either way, your doctor should be able to help after an exam.

It can be treated so that it goes away if it is diagnosed properly.  It would be best if you saw a professional to determine what is causing this rash.
