Sometimes: Fatal insomnia is very rare. Symptoms starts with insomnia in middle age. Fever, headache, attention and vigilance, memory, get impaired. Hypertension, increased breathing and heart rates, sweating, and slightly increased body temperature are also other sign or symptoms of this rare disease. The big unknown: Doxy is best started per recommendations of the CDC and others within the first 5 days of infection.  The benefits of delayed treatment in the post-acute stages are unclear.  Not alot of literature or research that I've ever found on delayed treatment.  The RMSF antibody titers may give your doctor some guidance.

Must go to doctor: No kidney infections can be life threatening in young women because of acute kideny infections, pylenophritis can sepsis shock and organ faillure. Back pain may suggest acute pyelonephritis if associated with high fever, chills or rigors. Need to see a doctor to do urine culture, antibiotic therapy, and to make sure there is no obstructive pathology like kidney stones. Hence, must see a doctor.

: That's an exceedingly rare side effect of doxycycline. If that was the cause of your headache, it would be continuous, not "come and go". However, you still should discuss the dual antibiotic treatment with your doctor(s). Probably one or the other antibiotic would successfully treat both problems, whatever they are.
