i've had a hard right testicle for years and everytime i have a yearly physical my doctor performs a testicular exam and says i'm fine. is that normal?

Hard testicle.: Does it feel different from the left testicle?  Has it changed from what you remember from prior?  Has the right testicle been growing?  If you're not satisfied with the answer you're being given, then perhaps a second opinion with a Urologist would be in order.  Maybe a testicular ultrasound.  Better to be safe.

i had a testicular injury in sept. after the exam doc prescribed diclofenac, i was fine by oct. i was fine for two months, but now i'm feeling that stabbing pain again in my testicle, which gets worse when i ejaculate. what do i do? no doc until jan?

: If there is no contraindication for using NSAID again, you can buy a NSAID over the counter such as ibuprofen, naproxen, etc. and use as needed until you can see your doctor in January. You can request a urology referral for a better care about this problem.

how does a doctor fix testicular torsion?

Rotation, surgery.: Non-surgical correction can sometimes be done by manually rotating the testicle in the opposite direction (outward, towards the thigh).  If this is initially unsuccessful, a forced manual rotation in the other direction may correct it.  Manual detorsion is successful in 26.5% to 80% of patients.  This is a surgical emergency needing prompt manual or surgical rx. But with good chance of success.

how do doctors determine if it's testicular cancer or epiditimitis?

Anatomy: Testicular cancer would show as a mass in the testicle on ultrasound.  The epididymis is near, but outside of the testicle. In typical epididymitis, it becomes inflamed and looks enlarged with increased blood flow.  It typically causes acute pain in the scrotum.
