Inappropriate: It would be very inappropriate and quite odd (in most cases) to have a patient masturbate as part of an exam.

No: There is no situation in which a doctor will milk the prostrate. However, the doctor may check the prostate if there is a risk of prostate cancer or enlargement. As you age your risk increases.

Hyperglycemia: The cause of insulin resistance is not fully understood. It is thought to be due partly to genetic factors, including ethnicity, and due partly to lifestyle.  If the body's insulin can not push the glucose inside the cells to produce energy for the body then you have insulin resistance in your hands . How do you know that ? You know that because the blood sugar level climbs above the normal.

Doctor visit: Many patients are being examined in physicians offices during this pandemic and, because of their office and waiting room policies, transmission of covid seems quite uncommon. There are no guarantees. However the physicians and their staff keep their masks on which protects you. If your needed appointment must be in person and if it's very important, I think you should make that visit.

: There are as many modes of presenting news as there are doctors. In general, they tend to suggest a biopsy at the earliest moment they consider a worrisome diagnosis possible, even when the issue is likely benign since they never want to overlook anything. The is especially true when early diagnosis leads to successful therapy if a problem is found.

WOULD ADVISE: Based on studies from england, lesions in spinal cord and just one in brain confer an 80% chance of clinically definite ms over 14 yrs.  Do not know your history or results of spinal tap, but i would feel it is best to go onto meds sooner than later, speaking generically.  Think this should be handled with a second opinion from a university ms specialist.  At this time, supplement vit d-3.

why would inflammatory bowel disases affect our eyes ? do all patients need an annual eye doctor exam?

Not common: Inflammatory bowel disease is an autoimmune process in which your immune system attacks your bowel.  This is sometimes generalized and you can get an inflammatory condition termed uveitis inside the eye(s).  Also prednisone is commonly given for bowel inflammation and has a side effect that can affect the eyes.  Best to get the eye exam annually if that is what your doc advises.

Not particularly: I am sure some may find it more of a chore than others. It is just part of the human anatomy and some will skip this part of the exam to save time unless a related question is raised. I consider it part of the routine male physical and  quite necessary for clearing athletes for sports and or teaching self exam to check for testicular cancer.

Accepted Answer

It depends on you: The type of blood & urine testing depends on your presenting problems.  But most often doctors would order a CBC with differential and a metabolic panel (different names or acronyms in different clinics).  Depending on need, i may well also order a thyroid panel, b12, vit d, and rbc folate (folic acid) levels.

what advice would you give to a fairly healthy 32yo female patient who, despite reassurance from her doctor re: her health, has a heart attack phobia?

Accepted Answer

Anxiety and phobia: You are describing worried thinking (about heart attack) which causes anxiety (emotion) which can result in behavioral avoidance (phobia) of something (eg exercise). How you think leads to how you feel leads to behaviors. What leads to a heart attack is coronary artery  disease, quite uncommon in a young person. Consult a cognitive-behavioral therapist about your worried thinking.
