See a gynecologist: Although this may be an ovarian cyst, other problems may be present. Having had your period a tubal pregnancy is less likely, but it sounds as if there is an ongoing inflammatory process in your pelvis.  You need to be seen, examined and tested appropriately, and this may include imaging studies (ultrasound, etc.)  good luck.

Not normal: Severe pain, normal?  Chronic pid isn't normal by any means.  Your doctor may not be treating you with medication that is working.  It's possible you have an infection other than one that responds to doxycycline. There are definitely bacteria and mycoplasms out there that are not going to get better with doxy.  Consider seeing a different obgyn, an id doctor, or at university, one with id ob/gyn.

Very uncommon: It is extremely rare - maybe not impossible, but nearly - to get PID when pregnant. If you refer to having PID and then becoming pregnant, the PID need sot be treated promptly just like when not pregnant (although some of the drug protocols might change. ) I hope this helps you.
