How far off?: The three weeks since your injury have certainly substantially reduced or eliminated immediate  treatment options. But rest easy. As long as the alignment position is minimal, there should be little affect on healing.  These cases are best assessed with a full set of immediate post fracture x-rays and immediate reduction x-rays.

See a doctor!: Falls are notorious for causing MSKL injuries and puncture wounds are high risk for infection. I strongly recommend presenting to a doctor for a thorough history and physical, incl. XRAY,  to determine the nature of your pain, evaluate the wound, discuss your tetanus vaccine hx, and provide the appropriate course of care. Home remedies help in some cases, but you need a doctor&#x27;s expert evaluation.

The question is:: whether these are actually lumps or growths--such as a Morton's Neuroma--or whether they are bony prominences which are common in the area you describe. A Talor's bunion can develop at the point where the 5th toe connects to the foot. Or sometimes this is simply a normal bony prominence. I recommend a visit to a Podiatrist or Orthopedic Foot Specialist to obtain the correct diagnosis and treatment

Gout : Gout is a possible answer as the first metatarsal joint of the foot is the most common joint to be affected by gout. It is possible you may also have plantar fasciitis. In either case, rest, ice, and if you are not allergic, an anti inflammtory like Motrin for pain. If the pain does not improve you should be seen by a physician.

Have cast changed: It is not uncommon that you are unable to move your toe, if it is the one associated with the fractured metatarsal. You have been in a cast for a month if you are having a problem the cast can be removed and the foot examined.

Leg muscle pain: When you walk you have to lift your foot and toes to clear the ground. This exercises the tibialis anterior and anterior muscle compartment. In some people this can result in pain due to muscle swelling. Therapy and strengthening exercises can help. In some cases of what is called 'exertional compartment sydrome' surgery is necessary for pain relief.
