Difficult to heal.: Under the best of circumstances, sesamoid bones are difficult to heal. Having said that, it is at times to determine if the sesamoid is actually broken. This is because sesamoids can be in two, three and even more pieces, naturally. It is prudent to x-ray the opposite foot to see if the sesamoids show the same mono, bi, tri or multi partite pattern. If truly fractured, it may need to be removed.

Which Sesamoid: The great toe will deviate towards the side of the remaining sesamoid in some cases and should be fixed if possible, rather than excised.  The 'tibial' sesamoid is the one towards the middle of the toe, the 'fibular' sesamoid is the outside one and excision of either will cause the toe to deviate away from the side that has been excised.  Look this up online if you need more information.

No weight for 2-4 wk: A sesamoid bone is usually removed for a fracture that doesnot heal, avascular necrosis (dead bone) or severe arthritis. There are 2 so  one out is ok.The incision may be on the side or bottom of the foot so it is uncomfortable for about 2-4 weeks. An orthotic may be required post operatively to make up for the loss of the bone and physical therapy to regain motion and function.

Fracture: It rare but possible to have a 5th metatarsal sesamoid 4.7% according to one article. I suggest you get an MRI to check if the sesamoid is fractured vs a possible pathology in the surrounding tissues or joint. Physical therapy is one option at this point.

Certain conditions: Pain due to chronic sesmoiditis or arthritis can occur in the hand and foot after trauma or in association with inflammatory diseases.  Initial treatment is conservative - ice, nsaids, unloading and behavior modification.  Steroid injections can be both therapeutic and diagnostic (if symptoms improve, it is likely to be the source of pain).  Sesmoidectomy should be the last resort.
