It's easy: Check at your nearest hospital for doctors  on staff. Then check those with offices close to your home. Then check to see if the doctor accepts Medicaid patients. Unfortunately because of poor fees and excess paperwork many good doctors cannot take this insurance. Look for a family doctor or internal medicine specialist depending on your medical history.

Braces: Hopefully you will have a dental plan and your orthodontist can make a referral. Get as assessment for her treatment. All the best.

Yes.: You are referring to the fungus coccidiodes immitis (cocci for short) which causes the lung disease coccidiomycosis sometimes referred to as valley fever.  It is common in the central california valley and southwest. The infection is often mild and not diagnosed, but it can leave nodules, scarring or cavities that can appear on chest x ray. It can disseminate and cause serious disease in some.

No problem: Not an issue, nothing to worry about. The atmospheric pressure changes are no more (probably less) than everyone experiences in a typical airplane flight -- and those changes occur in a few minutes, compared with many hours while driving. If you have a problem with blocked sinuses or ears, there could be minor symptoms, but nothing more -- and even these can be prevented with decongestants.

i live in arizona (phoenix at the moment) where the coolest is 95 right now. i used to walk 3x (30 mins each) a day and now i can't. should i just walk in the heat and bring water?

: If you  have access to an indoor mall with air-conditioning, it would be safer to walk inside. Drinking water will help but exerting in a hot environment carries the risk of hyperthermia. 
You may try a different way to exercise, e.g. rope jumping, treadmill,  or elliptical, in a cool room.

Perianal Abscess: You may have a perianal abscess, you can visit a general surgeon or a general surgeon who specializes in colorectal surgery. The MD will give you a full exam, and possibly incise and drain any infection and treat you. Arizona is a pretty big state, but if you are on the internet you can use Google to find a surgeon near you. Good luck.
