Biopsy with intent!: An excisional biopsy is a biopsy where the lesion is also excised or removed.  

When people here the word biopsy, they usually think that just a small sampling of the lesion will be obtained.  However, there are times when its more appropriate to remove the whole lesion and send it to pathology.  This  would be an excisional biopsy.

No: I am betting that you are asking about enlarged lymph nodes. Cancer in nodes is obviously very abnormal. If the nodes are simply palpable and maybe a bit tender, leave them alone.

Depends: What was excised? How much tissue was removed? Was it under local anesthesia or general? You only need to rest as much or as little as you are comfortable with. A small excisional biopsy of a skin lesion, under local anesthesia would not require any rest.

Usually unnecessary : Nonsurgical needle biopsy is the standard for diagnosing mammographic abnormalities.  Once the benign diagnosis is established, it is usually unnecessary to surgically remove a benign fibroadenoma.  However, if it is followed and growing, excision is often recommended to prevent further growth and exclude rare "locally aggressive" fibroadenoma-variants called phyllodes tumors.

Depends: Typically dermatologists do a shave biopsy.  They are appropriate when well-trained people do it (usually derm - most general surgeons and gp's do not have that specific training.) the problem with excisional biopy is that it may complicate the required surgery if it is a melanoma. Like most things - go to a qualified specialist.

Excisional biopsy: It can be quite important to see the entire node as the "architecture" is key to the diagnosis. Taking the node out is the "gold standard".
