LPR: You have what sounds like laryngopharyngeal reflux(LPR). This is a common condition from reflux also known as GERD. Man patients only have LPR symptoms. Aggressive Rx with acid suppression is suggested with a PPI ( proton pump inhibitor) such as omeprazole- it may take 4-8 weeks to get better.

Tampons: Are you referring to  interior  of vagina, labia etc? Do you mean you wear a tampon even when you don't have a period. The cotton from tampon can be rough and may inflame tissues if you  use them "a lot of the time." Take care.

Tricky: dermatologic conditions are hard to discuss without seeing them. and the iris has colors and spots normally. They even slowly change over time.
certainly an eye exam by an MD would be able to tell and find out many other useful screens such as vision and eye pressures.

You : You could be dealing with a cystic lesion, lipoma (fat filled lesion) , or even the start of an infection. There is also a type of nodule in people affected lupus that starts under the skin and is very tender to the touch. It is extremely difficult to say with out a more detailed history and physical. My advice is for you to schedule a visit with your primary care provider, dermatologist, and/or rheumatologist for an evaluation and treatment.

See Dermatologist: Not sure what happened and qualifications of the therapist. Best if you seek advice from a dermatologist. In the meantime, stay away from the sun, protect with sun screen/block. Use cetaphil as a gentle cleanser and may be helpful to use a topical cortisone to sooth any inflammation. And all times, keep the skin moisturized and not let it dry up or scab.
