i am 33  old female with upper epigastric pain under xiphoid process. better in the morning, worse by evening, i have mid lt back pain near spine. i have had neg. abd. ct, upper endo, stool test, lipase and abd ul 4 months ago. symptoms still persist?

Upper abdomen: Your tests appear to have ruled out gastritis,peptic ulcer and pancreatitis. Has the gastroenterologist ruled out common duct stone, cholangitis, hepatitis?  Is another gastroscopy been considered to look for an occult gastric ulcer?

how do you differentiate the painless, central small lump, just under the sternum as xiphoid process or epigastric hernia?

Hernia or not?: Biggest way to differentiate is does this area change with flexion of the abdominal muscles.  i.e. lay flat and lift the feet up off the bed (a leg lift).  Does the lump come out?  If it does, it is a hernia.  How about squat down and lift something. Same rule applies.  Now, without a large lump it does not necessarily mean no hernia, but if it does, that is the answer.

Soft tissue swelling: means exactly that, swelling.  If they were specifically looking for a hernia and could not see it on ultrasound, most likely it is not a hernia.  You should see a surgical specialist just to be sure.

See your doctor: You definitely  need some testing done, would suggest an ultrasound and blood work including a   standard set of blood test. Major possibilities include Gallbladder,  pancreas or stomach ulcers. The above test will point the way to further investigations.

an empty/hollow feeling in upper abdomen with what feels like something is flip flopping. when i point to where the feeling is its right below my xiphoid process. i feel this even when i'm not hungry but food does suppress this for a few hours.

Stomach discomfort: Is this new? May be just hunger pangs or may be gastritis /reflux( though usually food may makes it worse) . with or without H pylori a bacteria.    or esophagitis  or gallbladder, though usually food does make that worse.  Antacids like Maalox, ranitidine Prevacid may help .  For H pylori you need antibiotics.  for gallbladder need ultrasound to diagnose. f/u/ w/ doc

Gallstones: Can commonly be a cause of gallbladder irritation or inflammation (cholecystitis) resulting in upper abdominal bloating or pressure discomfort, nausea ; sometimes vomiting. Often gets worse after eating fatty foods. See a general surgeon for evaluation as you may benefit from gallbladder surgery. If this is an OB ultrasound during a pregnancy, then ask your OB doctor for instructions first.

Possibly: A heating pad may be used provided its not too hot, that the user is not under the influence, and that the pad actually helps.  If symptoms worsen, call your doctor.

Anatomical sites: There is no real difference except for the anatomical site- chest pain can be due to several causes including heart/ lungs, but epigastric pain usually implies pain coming from stomach or esophagus.
