Listen to your body: It could be a histamine response since you have allergies. Vitamin c with bioflavanoids, boswellia, and curcumin are natural substances that block histamine and inflammatory substances that can cause intermittent pain. Advil (ibuprofen) that you use  is anti-inflammatory. Does it help at all?

Go to a DO : If you've already gone to a doctor and received your cardiology workup, it might be wise to go to an osteopathic physician for a second opinion, to help determine the origin of your left arm, and shoulder pain. S/he can do a full osteopathic musculoskeletal examination, and determine if your left arm, and shoulder pain is structural, or in some way related to another issue.  If it is musculoskeletal origin, she/he can do and osteopathic manipulative treatment, and relieve that pain.

LAE: Left atrial enlargement may occur as a result of abnormalities of the mitral valve.  Once this has been repaired or replaced (sounds like mitral stenosis) the atrium may or may not return to normal size, but the heart should be functioning better.  Best that your sister or you discuss this at length with the cardiologist and cardiovascular surgeon.  Hope all goes well.

Mgt/workup: It is possible that the right sided heart problem is related to a change in the anatomy on that side. The leaky valve makes the heart work more with each beat. Since you have experienced leg swelling, the regurgitation may have been significant enough to allow back up of the blood, resulting in leg edema. The valve may be repaired to alleviate this cause. It is important to follow up.
