HIV: lung ressection: If you are being treated with current HIV medications, and have not developed AIDS , weight gain from your current weight is going to be difficult.  Best is to meet with your current healthcare provider and discuss possible methodology to increase lean body mass.

Could be several : There could be several reasons. It is  less likely spinal FAS a typical scoliosis which is usually a deformity not associated with pain in this region  unless it is potentially a large scoliosis.  I would be concerned that this is not an ulcer or related issue and you should have this evaluated by your doctor.

Air bubble in chest : Pneumomediastinum or air in the chest can come from lungs, trachea or esophagus. It can happen from deep sniffing, drug abuse, and heavy exertion. I see a number of teenager with lot of air in the chest from unknown cause. Air can also come from root canal. Air bubbles can also mean infection. It may be sign of grave disease process or it may be totally benign requiring simple observation.

had nissen done over 10 yrs ago.  it has loosened and slipped into chest cavity. can it be taken down and reversed and can hernia just be fixed.  ?

Not So Simple: Please return to see your original surgeon or find one very experienced in this surgery--the "second time around" can be much more difficult than the first operation. You may require pretty extensive pre-op testing (24-hr ph, manometry, barium swallow, ct) to plan out the operation & decide how best to fix the problem.  Good luck.
