Biologics: No it is not offensive but an emollient lotion is not effective.  Talk to your friend about the use of biologics such as enbrel, humira, Remicade (infliximab) as these medications treat both skin lesions and arthritis.

Steroid cream: Lichen sclerosus is poorly understood as far as a cause.  It does respond, however, to high potency steroid cream.  The most commonly used cream is called clobetasol.  It is probably best used as an ointment and is available only with a prescription.  Talk to your ob/gyn!

Soap &amp; water: should be sufficient to clean under foreskin once it has been retracted fully. Phimosis, tight foreskin, could be cause of inability to easily retract foreskin which would make penile hygiene difficult. Need to see your Dr. or urologist if foreskin does not retract easily. This might require steroid cream or even surgery. Surgery would be either circumcision or prepucioplasty/dorsal slit.

Depends.: Everyone's skin reacts a bit differently to various things that are applied to it. Both products are very good, in my experience - but you might prefer one over the other, depending on your individual preferences. The only way to find out is to try both. Some pharmacies keep "demonstrator" bottles of popular lotions on the counter, so that you can try before you buy.

Lots: Among some of the available otc natural emollients available for dry skin that are rich in omega 3 and 6 are emu oil and flax and hemp seed oil. Of sourse eating oily fish like salmon, and nuts and enriched eggs are good too.
