echocardiogram: an ultrasound, shows the actual structures such as the heart muscle and the valves and with a doppler the direction and velocity of blood flow.

would a echocardiogram report a right bundle branch block? because it was normal, but my electrocardiogram did report a rbbb, false reading?

Right bundle branch: We share similar EKGs and I have no heart poblems.  According to a recent article in the European Heart Jounal the prevalence of RBBB that is normal variant is 4 percent for men and 2 percent for women.  RBBB does develop in people with lung problems, especially with COPD or Pulmonary embolus. It can develop in some people who have other processes that effect conduction.

Echo: Heart failure with normal ejection fraction would most commonly be due to some structural or functional abnormality in the heart, it’s valves, or surrounding structures. To start an echo would give the best chance at visualization along with a good physical exam. 3D echo would be even more helpful in some cases if available, and stress echo may be needed to expose certain abnormalities.

: Most medical technicians and assistants are very familiar with anxious patients and will adjust procedures to enhance relaxation and improve cooperation.  Your emotional state should not impact the outcomes of your described tests.

i've had over 100 electrocardiograms. 2x echocardiograms. 2x 24 hour holder monitor. all normal. ectopic beats/pvc's. got new smart watch recently and when i feel these "skips" it's telling me i have atrial fibrillation. dr's missing this?

: Given the testing already done I would throw the "stupid" watch away. Those people who have real treatable rhythm disturbances will show up on a Holter monitor if nothing else. Find a life coach that can teach you stress reduction, relaxation skills. The stress of this is probably part of the problem.

See below: Echo checks for wall motion ie prior heart attacks and contractile function of the heart. Can have abnormal rhythm   like atrial fibrillation  or cardiac extra heart beats with normal echo.

Non-invasive CV eval: Non-specific EKG changes during stress testing are common. Stress imaging (ECHO, nuclear or MRI) carries more weight as it allows the doctor to assess the flow of blood into different parts of the heart muscle and also assesses the 'Ejection Fraction', which is a measure of the strength of heart contractions, above 55% being normal. Coronary calcium scores assess heart artery blockage risk.

Heart function: Echocardiogram & EKG (ECG) are 2 completely different things.  An echo is an ultrasound - it displays a picture of the structure of the heart, its movement, pressures, and blood flow trajectory.  This helps your doctor determine any anatomic or functional problems with the heart.  An EKG measures the electrical voltages of the heart. This provides info on possible heart problems.
