Difficult to say: Heroin use in the first trimester will either cause a full miscarriage or have minimal effects at all.  This is essentially true of all toxic exposures in the first trimester that tend to kill the baby or not affect it at all.  Heroin use for the rest of the pregnancy will likely cause addiction in the baby, stunted growth and increase risk of infections such as hepatitis, hiv, syphilis.

Maybe: The baby will certainly do better than being on heroin through delivery with a guaranteed withdrawal syndrome.

Pregnant w/dermoid: The dermoid itself will not have any effect on the baby or the developing pregnancy.  That said, given the size, it runs the risk of causing ovarian torsion or labor obstruction.  Discuss the pros and cons of removal vs. expectant management with your OB.  Your dermoid is really reaching the size where removal after the first trimester may be recommended.

Very accurate: Ultrasounds tend to get more inaccurate as your pregnancy progresses.  First trimester ultrasounds are more accurate than second trimester ultrasounds, which are in turn more accurate than third trimester ultrasounds.  First trimester ultrasounds can reliably predict gestational age within 5-7 days.

The majority of patients who experience spotting early in pregnancy will go on to have a normal pregnancy, and deliver a healthy baby.
