Onset of infection: Sounds like a throat infection is setting up. The painful nodes are a good sign that your immune system is responding to try and fight the infection, whether it is viral of bacterial. Since it doesn't sound like you are getting much better after 5 days, you need to be checked for strep throat and take antibiotics if you are positive. The other outside possibility at your age is Mono. Get seen!

wife (38) had ear & throat pain+fever. pcp found severe throat infection- prescribe amox.10 day course over 10/3. ear pain+lowfever is back.next step?

Treatment failure: Symptoms of an illness that will respond to a particular antibiotic usually start to improve within 3-4 days. If the appropriate antibiotics were prescribed, and they were taken as directed for the full course, symptoms should not return. Relapse of symptoms is generally regarded as a treatment failure, although it is possible that another infection has developed in the interim.  Revisit your PCP!

I would recommend: seeing your dentist. This could be related to an infection or cavity in a tooth on your right side. If the dentist doesn't find anything and it persists, please see your own doctor or return to the UrgentCare or an ER for further examination and possible labwork or Xrays.

You should be: examined by your doctor. This could just be a viral infection, but it could be an ear infection, throat infection, or sinus infection. For tonight, if you can't see a doctor, you could take ibuprofen or acetaminophen, if you have no other medical contraindication to doing sol.

Unlikely thyroid: Thyroid problems seldom cause pain like that.  Even if you had a nodule or a thyroid cancer, they may grow and cause a lump, but they do not usually cause pain that radiates up/down like you describe. See your doctor.
