Scapular pain: There are a number of causes of scapular pain. The most serious is referred pain from the lungs or heart.  Musculoskeletally the discomfort can be from an irratated nerve, muscle spasm, bursitis. This should be evaluated by a physician.

Yes: You may have strained the muscles in between your ribs from coughing repeatedly. If the pain persists you may want to see your pcp. Sometimes this pain may be from the pleura (the lining around your lungs) as well.

Muscle strain: There are several joints and intervertebral discs in the spine.  These can wear out over time leading to pain.  Degeneration of the joints or discs can cause back pain.  You can also get back pain from a muscle strain.  In some cases pain in the middle of your back could be related to chest conditions or your heart so be sure to see you physician.

Many Possibilities: Depending on how long it's been going on, i would consider an evaluation by a spine specialist.  Sometimes it could be a muscle imbalance, early onset arthritis or scoliosis.  Also could be a pinched nerve/herniated disc.  Either way it might be good to figure it out and get appropriate treatment.

Could be arthritic : Could be arthritic changes or degenerative disc disease. When you bend to the right, the nerve roots get irritated.

Recurrent back pain: this is probably a recurrent back injury. However, you have to be careful. Particularly, if it continues to come back. I would worry about a kidney stone or infection that can sometimes mimic muscular back pain.. also you have to worry about more serious causes of back pain like spinal cord stenosis.if you are having recurrent back pain for three months or longer go to the doctor
