husband has dull pain in perineum after ejaculation. has had on and off perineum pain when not ejaculating, too. burns when he pees but only after drinking alcohol. any ideas on what this could be? could it be affecting fertility?

: Genitourinary pain can be caused by infection, trauma or other possible causes of inflammation.  It merits an urgent visit to one's doctor or to a urologist.  A careful history and physical exam, with a urinalysis and blood tests including  a CBC, kidney tests and PSA (prostate specific antigen) will usually reveal the likely cause and appropriate treatment.

: It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. You may take acetaminophen for the pain, no more than 4 g in a day, no alcohol, If that does not help it would be prudent to see a doctor. Wish you good health!

i have a dull aching in my perineum. psa results were fine, but i've had hemorrhoids and colorectal pain before. do i need to look into getting a colonoscopy or could it be muscle related? not sure if i've injured nerves down there with exercise etc

Rectal exam is in or: Dull perineal pain is often due to an irritated or prematurely enlarged prostate. Often, in young men, this is due to extended time on a bicycle seat. Digital rectal exam by an experienced person can often identify the problem and PSA, though useful for detecting early prostate malignancy, would not be affected by a condition such as yours. Next stop - colonoscopy.

ive had intense tingling, aching at my perineum/anal area and sometimes buttocks since sexual play (no penetration) with a guy 8 days ago. began feeling this 48 hours after contact. no rash/sores yet. been taking valtrex as a precaution. herpes?

: Herpes occurs as eruptions on/near sex organs and may give some symptoms similar to what you describe. Wait 2-3 days . If no eruptions good chance no herpes. next time pl take precautions.

Could be cause: Large varicoceles can certainly cause pain and an indication for varicocelectomy if present. Moe likely cause of pain if you get relief by wearing jockey type , spandex or boxer-brief underwear rather than boxers.

See urologist: sometimes uti's present like this. You need to be evaluated and possibly treated by doctor.

Prostatitis: Your symptoms are consistent with a chronic prostatitis. Most common type is non-bacterial and treatment can be more complex often requiring patience and trial and error with a variety of options. That said, the finding of klebsiella on semen/prostate fluid culture suggests a chronic bacterial prostatitis which requires long term antibiotic treatment and further assessment of lower urinary tract.

: Holding 1500cc in bladder is a bit outside normal. Are you sure?. Try emptying your bladder frequently and Sitz baths for a week or so with otc Ibuprofen

Head pain: If there has been no injury, such dull headache without other symptoms is nonspecific but not unusual. If it resolves with no treatment or a little Tylenol (acetaminophen) it most likely isn't anything serious. Muscle contraction ache is a  common cause. If headache persists or worsens, see your physician for evaluation.

Needs eval: Multiple causes, needs physician eval promptly.
If sudden onset,  seek er immediately for possible torsion (twisted testicle) that can impair blood flow and result in loss of testicle.
Other causes: infection, std, hernia, cancer, and benign disease.
