Corneal lining: The inner lining of the cornea consists of cells which pump fluid from the cornea keeping it clear.  When these cells are deficient or incompetent, the cornea swells with fluid.  This when spontaneous is called fuch's.  Surgical replacement of the cells from a donor can restore clarity in a procedure called dsek.

Your symptoms: generally will dictate when you are ready for a corneal transplant. Symptoms of decreased vision, glare, light sensitivity etc. If the corneal swelling is to the point where I worry about permanent corneal scarring, then I usually advise the patient towards surgery sooner rather than later. Avg numbers are about 640 for corneal thickness before surgery. Good luck.

in a pt who needs a dsek with cme secondary to an ill fitting aciol that was placed over 5 yrs ago, would you remove aciol and fit w contact lens?

Depends on staus of : The eye. Is there support to place an iol posteriotly or can one be placed in position with sutures. Also, depends in what you and the surgeon are comfortable doing.  A contact lens is an option.

A lot of info here: There is a lot going on here, but sounds like some of the common problems with such a procedure. You will not have good vision as long as you have cme. Sometimes with dsek you can't get 20/20 but worse than 20/40 is unusual. If you have a multifocal iol with a dsek, i suspect that could give diminished vision as well.

That : That may be the case, but glasses prescription should make this somewhat better if though you have the toric lens implant.  Ask your cornea surgeon what the limitations are.

i had cataract surgery and corneal transplant (dsek). the corneal slipped and a "refloat" was done. it had "scratched" from stretching the cornea and vision was blurred. how many corneal transplants can be performed on the same eye with positive results?

DSAEK : Dsaek can be repeated with good results.  It is a relatively new procedure so there is not a lot of research on multiple repeats.  Provided that there is not a lot of cornea scarring then the vision should be quite good.  That being said, vision expectations after dsaek should not be 20/20.  The vision is frequently a little less than that (average 20/30-20/50).  I recommend visiting a cornea specialist who did fellowship training in corneal transplantation.  If you have significant scarring then you could benefit from a full-thickness cornea transplant.  You also need a full eye exam to make sure that your retina and optic nerve is healthy.

Diet?: There is no best diet.  No restrictive plan ever works long term.  A "diet" usually means that the patient is going to go off of the "diet" when they have lost the weight.  Guess what happens then?  You go back to the foods you gave up...And you gain the weight back.
Most patients then feel like failures, and it's not their fault! reducing
food portions, exercise, support for change will help.

Yes: Excess weight is a common problem in children.  They frequently follow the example of their parents who are often overweight as well.  Children can go on diets and should be encouraged to do so  if they are obviously gaining weight.  Enlist the aid of your pediatrician and a good nutritionist.

A low calorie diet : As with many diets it has no long term results. Instead eat healthier with more vegetables and smaller portions.
