Cough: I think that drinking something cold can indeed make a cough worse due to the close proximity of of the esophagus to the trachea. Cold typically causes structures that have the ability to constrict or narrow. In the airway this narrowing or constriction can lead to bronchospasm which leads to shortness if breath/ coughing/ wheezing.

Fluid intake: Drinking fluid with the meal may decrease appetite and dilute the digestive juices. Drinking afterwards I'd okay, as is drinking fluids throughout the day to stay well hydrated, especially in hot weather and air conditioned environments.
