my 85 year old mother took her first dose of 30 mg duloxetine last night. this morning she was in tears due to insomnia, upset stomach and extreme muscle twitching during the night. it was prescribed for nerve pain but didn't help. should she stop?

: At least 30% of patients taking duloxetine can suffer nausea and vomiting.  Do not take any more.  Notify prescribing physician.  Make certain that her pain generator is indeed "nerve pain".  Obtain neurological consultation.

been taking citalopram for a few months. my doc added mirtazapine for my sleeplessness recently. im taking both; experiencing muscle twitching. help?

Celexa (citalopram): This could be additive effect on both medications. I'm not sure about the dosages. Please consult your prescribing physician.

Benign: You   may   want  to see  a  sleep   medicine   specialist   to  rule  out the   possibility of   sleep   apnea.
There   is   a  lot of   causes   of  muscle  twitching.  It  can   involve    different   muscles.   It  is   likely  benign   and    may be   caused   by the   electrolyte    abnormalities.

Lose the meds.: I thinks that if you are otherwise health its time to stop the meds.Lower the gabapentin slowly. You may have sleep anxiety. As for the muscle twitches they may come from fatigue but see a good Internal Medicine physician

Vitamin deficiency?: Vitamin B12 deficiency.  If so, there should be other symptoms such as decreased touch and vibration sensation in the  feet and decreased reflexes.  Vitamin B12 deficiency can occur with various medications, intestinal problems, and malabsorbtion of nutrients.  The  diagnosis is easy with  an exam and  common blood tests.  Other causes could be in the spinal cord.  Go see your doctor!

Difficulty falling: asleep is often associated w/anxiety while waking up early and not returning to sleep is associated w/depression.  Periodic waking is associated w/overall light sleep.  Please see you doc who may order a sleep study.  A psychologist can help w/stress management.   Natural treatment includes melatonin supplement but w/professional guidance.  Stop all stimulants after noon. Insomnia not fatal.

Anxiety: Sounds like you could be suffering from anxiety. See a psychiatrist and/or therapist for appropriate stress management and coping techniques. Try to reduce stress and workload. Take time to yourself to relax. Learn meditation. Take time away.

Can be many things: First thing to do is hydrate well and see if that makes a difference. If it does not, you should see your primary to check for basic causes, from continued dehydration to certain levels in your blood. If it continues, check with a neurologist or physiatrist to get an emg to rule out anything worrisome.

In some cases, yes: Depending on cause, simple muscle twitching due to myofascial strain, can be helped by massage.  If the twitching persists, then other causes need to be sought and you should consult a physician.
