Depends: Depending on who is actually performing and reading the ultrasound will determine your actual risk.  A highly experienced maternal fetal medicine doctor will have a very low risk of missing a baby with down syndrome.

Problem in chromasms: Babies are supposed to get 23 chromosomes from mom &  23 from dad at he point of conception.In ds babies, an extra # 21 chromosome joins the mix for reasons that are not clear.As the egg grows the extra becomes part of every cell & baby is born with ds & all its problems. We know there is risk of ds for any pregnancy & somewhat higher for mom <17 or > 40. Genetic counseling could give u specifics.

Yes 80/20 diet: Where 80% of your diet consists of low carb, high lean protein and plant or fish based fat  and no alcohol and 20% is rather unhealthy, meaning higher carb diet. If one can follow that (and I have many patients who are on it for maintenance) for most part, they can maintain their weight. So about 2 meals and 1 snack healthy and 1 meal is considered the "guilty pleasure".

Antibiotics: This depends on extent and individual case. Oral antibiotics depending on bacteroa pedant can help as well as anti-inflammatory cream such as Elidel (pimecrolimus) or pro topic ointment. If possible the area needs to be removed in part or in whole.Laser or even liposuction can be used to remove the damaged inflammd glands.

Pubic hair: Insure you use fresh, sharp blades vice dull blades. If hair is &gt; several mm long – then trim before shaving. Wash area before shaving – warm water helps to soften hair. Use a lubricant when shaving. Avoid using products that contain alcohol or are scented.  Shave in direction of the hair – not against it. Be gentle when shaving – don’t push down too hard. Moisturize skin after shaving.

Lose 58 lbs.: A reasonable wt loss plan would be about a lb a week ( a deficit of 500 calories per day) through some combination of exercise -600 calories / hour  - and diet.   This will be a long road, but is entirely do-able.
