Yes and yes : Prolonged erection can cause post erection pain in penis..happens most of time when no sex happens.  It has been called the "blue balls".  But if you ejaculate some pain can continue but with no consequence .  varicocele is a collection of veins most always on the left and for the most part does not bother the other side.  It may hang down more and get a little more banged up with vigorous sex.

ED EVALUATION : From your symptoms and list of Meds I would sugest a second urology opinion. Recheck labs Especially TFTS/TESTOSERONE level. Recheck testicular ultrasound. Consider empiric trial of viagra/Levitra/cialis

i know that masturbation/sex cant cause or worsen varicocele. but why? can i have explanation about it? when near ejaculation, a lot of blood flow to testicle right? so how ejaculation not affect varicocele? just curious?

Varicocele: Probably sexual release does not increase blood flow though the varicocele, at least not much; and even if it did, that would not be harmful. Varicocele's do not develop because of blood flow abnormality, and increased blood flow does not make them larger or worse in any way.

i felt pain in my left iliac during last ejaculation. currently, low libido and pain before possible ejaculation. had varicocele, hydrocele surgeries 15/10y ago and stone kidney 4y ago. have no pain normally, no swelling, no sexually tran. infection?

: I don't think that the varicocele is an issue here. I doubt that the pain actually emanated from the kidney. The other symptoms were probably caused by anxiety. If you experience any further urinary or ejaculation discomfort, consult a urologist for evaluation and advice.

Normal Life: There is nothing to do at all.  This should not in any way interfere with your fertility or sexual life.

: Varicoceles are harmless, except if they get large then can be uncomfortable or unsightly. Probably nothing need be done in this case, but see a urologist if you remain concerned.
