Well....: Turns out that exercise does indeed burn calories  - about 600 calories per hour for vigorous exercise.  ( 3600 cal is a lb of fat ) That in turn leads to sweating, so some of what he says is true.  However, calorie loss through urine is negligible unless of course you are taking one of the newer anti diabetic medications which poison the kidneys ability to reabsorb sugar.

I do not know: What did they tell you in the ED? Did they do any tests.  A severe UTI can cause blood, kidney stones, lacerations, prostate issues, nephritis, cystitis, urethritis, etc. etc.  You need an evaluation if the Blood continues and you were never diagnosed yet.  Good luck.

Diabetes? : If she is having frequent urination, excessive thirst or family Hx of diabetis you need to consult her pediatrician, blood &amp;  urine test should be done.

Urine smells: Well, urine has an odor that should be distinguishable from sweat. Also it is yellow. And sweating in just the groin area would be unusual.

Chest x ray: If you have not already had a chest x ray i certainly would get one. If you have a poor appetite and i losing weight you need more of a workup.

That's a very...: ...colorful description but, even if I could tell you what's causing it, I couldn't treat you.  Foul smelling urine or vagina discharge, pain in the low back and sweating are red flags for something that needs medical attention.  Kidney stones, bladder or kidney infections and bacterial vaginitis are possible conditions that need to be ruled out or treated.  Best wishes.

Balanitis: Yes stagnant urine can be source of foreskin irritation  need better local cleansing. May need circumcision
