Should not: After treatment the rash should not flare. The best treatments are Permethrin 5% cream and / or oral Ivermectin depending on the severity of the case.

Scabies: It is very possible that you did not have scabies at all. If your practitioner did not scrape the lesions actually see the mites, the possibility of scabies is lessened. Sometimes the medicines given for presumptive scabies can be quite irritating in their own right. That means that you should definitely see a dermatologist and get this assessed properly. You may or may not even have scabies.

A couple thoughts: If you overuse scabies medicines it can cause skin irritation.  You may itch for several weeks after tx because your skin is still reacting to the mites /feces.

Side Effect: The topical scabies treatments do actually cause itching because they are irritating to the skin itself.  You can try topical cortisone 1% as well as zyrtec (cetirizine) antihistamine as needed for itching, then retreat with scabies treatment if not resolved within a week.  Good Luck!

Unlike head lice, scabies has not developed resistance to its established treatment, prescription topical Permethrin 5% also called Elimite or Acticin (permethrin) (here in the United States).

Scabies: Permethrin treatment needs to be repeated one week after the initial treatment.  There are oral treatments for scabies, like ivermectin.  However, the itch of scabies can last up to 6 weeks after proper treatment.  As long as you are not seeing new lesions, the scabies is treated and the itch will eventually resolve.  A cortisone cream can help with itching but not on days using permethrin.
