Keep exercising: I have followed this whole business for decades and am not persuaded that there is any reason to treat this particular lab value if you are otherwise fit, lean, and healthy. Many -- perhaps most --physicians are with me. Stay fit and be glad for good health. Would welcome colleagues' input.

You can but: According to most clinical studies, garlic was not proven to have any impact of cholesterol.  All the best!

Should be same.: Milligram for milligram, they should all be similarly effective.  That being said, there's more regulation and quality control placed over medication (NiaSpan) compared to OTC agents so the dosing of such might be more accurate and reliable, technically speaking.

Leave it alone: Some of my colleagues here will disagree. I do not believe in treating isolated hypertriglyceridemia. After fifty years, nobody's showed any real evidence that this is a risk factor for anything beyond being perhaps your body's way of asking for more exercise. Why not try getting some fitness stuff going instead? You're only 35; you could be far fitter than your peers, buffed up, easily.

Need more info: It would be better to reduce your intake of saturate fats, carbohydrates and lose weight to better control triglycerides. 
Make sure to consult a kidney doctor to make sure that your kidney function does not get worse.

It's called flushing: This reaction to Niacin (nicotinic acid) is due to prostaglandin release (chemical mediators), which gives you the unpleasant sensation.  It is a known side effect, it doesn't interfere with the cholesterol-lowering.  Ways to minimize this include taking an Aspirin before-hand, or taking your Niacin with food.

Yes: Unfortunately, neither diet nor exercise has been found to lower lp(a) levels. Lp(a) levels were lowered by 21% in patients taking 2 grams per day of l-carnitine for six months, Aspirin and Niacin are also known to lower lp(a ).

May be: Niacin may improve the lipid profile but it has not been proven to improve chances of heart attack or stroke. Also, they have several side effects and is hard to get people to take them long term
i rather you do more exercise (5-6 times/week), 
be in the best possible shape, loose a few pounds if you are overweight. That will increase the good cholesterol (HDL) and lower the bad. Your tg are ok.
