Imperforate hymen?: If you mean you have never had your first period, this likely means that your hymen has no outlet at all and the blood from your period can't get out.  See your gynecologist right away.

Hard to say: Blood is usually a reflection of tissue injury. Where/what was injured in that area is hard to pin down without looking. Perhaps you can position a mirror &amp; light in a way you can see. Most such scratching produces a shallow tissue tear that self heals. Just keep the area clean.

: Worry not. Only you know whether you have had intercourse or not. Many causes exist for a non intact hymen. Fret not my dear!

Apple cider vinegar?: If you are using apple cider vinegar for housecleaning or food preparation then great. If you are using it vulvo-vaginally then STOP. Your itching may be due to a yeast infection that the vinegar will only make worse. If there is no relief from the itching when the vinegar is discontinued then see a GYN MD to be checked for an infection.

Need to see a doctor: If the itching is just from sensitive skin or dry skin, such as eczema, then one can use an over-the-counter cream such as cerave, cetaphil, or eucerin. Adding Hydrocortisone cream for 1 week will help. Taking otc antihistamines such as Claritin (loratadine) or zyrtec helps too. The itching may also be due to an internal disorder, such as liver problems or intestinal parasites, so one should see a doctor.

There : There may be a few different things going on. The fact that your skin constantly itches may means you are suffering an allergic reaction. The hard part to figure out is what is the cause of this allergy. Medications are often the cause, and one or more of your depression and anxiety meds may be the culprit. It will take some work with your doctor to figure this out. Also are you using any new soaps, lotions, or detergents, etc? These are also common causes of itching and allergic reactions.
In addition there is a phenomenon called dermatographism. It occurs when the skin is scratched or pressed firmly, and results in a wheal or whelp as you stated. About 2-5% of people suffer from this problem, and it may or may not be associated with itching. This can be separate or in additon to an allergy that is causing your symptoms.
Make sure you follow up with your doctor in order to determine if your meds are the cause, and further evaluation into your itching.

"heat rash?": When you get this rash, get an appt so the dr can see it, and then the diagnosis would be made and the best treatment could start.  If only when sweating, possibly it is a type of minor heat rash that itches.  Stress can also cause itching, called stress urticaria, and meds such as Hydroxyzine hcl, which are fda approved for both anxiety and itching can help.  Good luck.

i am having a genital herpes out break what can i do to soothe my vagina im in a lot of pain and it itches bad?

: By far most important is to be on treatment with one of the oral anti-herpes drugs (valacyclovir or acyclovir), with expected improvement within 2-3 days. Until then, a local anesthetic ointment (ideally lidocaine) will help. Also make sure the diagnosis is correct; you should be in a doctor's care. Good luck.

toddler itches, breaks out during bath b4 any soaps/shampoo are applied. has reacted this way in other tubs. allergy towards something in water?

Mgt and workup: I would advise using a very basic soap such as white Dove soap. Avoid all perfumed products including shampoo. The reaction may also be to cleaning products and using natural cleaning products and rinsing the tub well prior to bath time may help. 
I would suggest an antihistamine and evaluation.
