Yeast infection?: The most common cause of such symptoms is candidiasis (yeast infection, thrush). If so, it seems more severe than average. Herpes might be possible if you are sexually active. Scabies rarely occurs in these locations, but might be possible if you have typical scabies elsewhere (hands, elbows, waist, etc). Allergy consult: The issue is complex and you should consult an allergist.  Do realize that even though we can usually control the rash, not uncommonly no cause can be found despite exhaustive studies.  In the meanwhile, try taking antihistamine 3 to 4 times a day to see if this may help.  The dose needed for itchy skin is much higher than what is needed to control hay fever but even then it may not work.

Monitor temperature, provide fluids, and apply a thin layer of antibacterial ointment twice daily to the areas until evaluated. Watch the scalp with a gentle antibacterial soap, avoid harsh chemicals.

For kids over 2 years, oral zyrtec or Claritin can also be used.
