Yes: A person can develop a significant allergic reaction to any protein containing substance.This can happen early on in life or later.Coconut oil or various components of this complex material can trigger allergies. Food reactions often trigger hives all over so if located only in the diaper area it may or may not be unrelated.

Fordyce spots.:  Fordyce spots are normal. I don't believe that Aloe Vera Gel or coconut oil would remove them.

Acne ?: Without a picture and details had to answer, however if it is acne, it will get worse with heavy moisturizer.  You may want use acne cleansers which have salicylic acid and avoid heavy make or lotion in this area.

Food sensitivity?: Some foods  may cause skin reactions (acne is controversial re contribution dietary factors).  A food and symptoms diary with doctor review may help sort this out

Direct is OK: You may apply the extract directly on the skin lightly and give it some time as it is suppose to help lighten the pigmentation in the skin.  However if the spot is really a deep dark it might not be as effective and everyone seems to respond differently. Make sure you continue to moisturize the skin while using the extract as well. There are some prescription topical medications which can be used for some cases and occasionally even radiation treatments. This can be a dangerous problem and do not rely on folk remedies like those you mentioned.
