Chemo limitation: Cancer cells or micrometastasis are unique cells which can continue to function and grow despite exposure to chemotherapy and radiation. Their dna adapts and finds new pathways for cell survival and proliferation. Sometimes it relies on the cells of the microenvironment to grow and flourish. We are getting better in understanding these pathways and finding targeted therapies to treat cancers.

Maybe: A positive attitude is just as important to treatment as is the treatment in my opinion. I think the body recovers faster if hope is not lost.

Sounds like: a serious problem.  Esophageal cancers have poor prognosis in general, because they are difficult to detect early and spread easily to adjacent and distant organs.

Need more info: Even with stage 4 head and neck cancers, some patient do survive.  Survival depends in part on the site of origin.  For nasopharynx cancers, the seer data (from the national cancer institute) show 37% of patients still alive 10 years after diagnosis.  Other sites of origin are better, or worse, than that number.  These data are for patients diagnosed between 1988 and 2001.  2012 data may be better.

Chemotherapy: Firstly, I am so sorry to hear that you are having these difficulties. What you are experiencing can be very hard to deal with, and having trouble sleeping makes it harder. Please talk to your care providers, including emails if calling is too difficult, to see what can be offered for medications. Listening to relaxing music or videos may be helpful, as might the caring touch  of a friend or pet.

Depends on Ca: A number of different malignancies defined as stage IV when mets noted.  Depending on the virulence of the cancer the initiation of treatment will be more or less effective in obtaining a response.  Colon or breast cancer delay in treatment by 2 wks will play a minor role in response or survival.  Metastatic pancreatic cancer or tumor metastatic to brain require a more intense and rapid initiation

The organ is Ok: Cancer involved different organs as isolated scattered tumor nodules. With a good/effective chemo, the tumor nodules shrink and can sometimes disappear completely. The remaining organ is healthy and functions normally once cancer has been killed by the chemo.

Verybad: Stage IV  of what organ ?     In general it is end stage terminal cancer , how ever some cancers  like lymphomas , testicular tumors etc  when detected late will respond well to chemotherapy.
