Dose is dependent: Dose of warfarin is very individualized. 
Diet, weight , metabolism of medications, co morbid conditions, other medications all effect INR. 
Safety of any drug is all relative. People with renal dysfunction or even dialysis are on warfarin.

i had an op in nov (had my prostate scraped). i'm still urinating blood and have lots of discomfort. is this normal or what advise do you recommend? the medication i'm currently using are: warfarin 5mg, mylan oxybutynin 5mg, bio-amiodarone 200mg, bil?

Need to examine: Being on Warfarin may be prolonging the bleeding. It would be prudent to contact your doctor of additional evaluation.
Blood loss can result in anemia and it would be useful for you to take oral iron, with meals.

on warfarin. fell yesterday.  calf swelled to the size of a grapefruit with jelly like fish raised marks. painful. burning.  er just took a cat scan and sent me home.  wat shld i do now? help!

If numbness/worse ER: A tissue bleed (hematoma) can be managed w watchful reassessment (could measure the circumference) with analgesics/warm compresses.  If the swelling worsens, pin gets worse, or numbness in foot develops need to return to er to rule out a compartment syndrome (compression of tissue, blood vessels, and nerves in a fascial compartment like te calf).  This can occur if ongoing bleeding on warfarin.

Depends: Great question; if you are on the medication longterm most healtcare providers check protimes/inrs every month.  In the most recent guidelines, however, some organizations are suggesting that every three months may be just as good.

The dose that works.: There is no exact upper limit to Coumadin (warfarin) although the most I have seen someone need is about 12mg daily.  Because Coumadin (warfarin) is so closely monitored with blood tests the right answer is the limit is the dose that gets you to your goal inr.  Some people are genetically "coumadin resistant" and need much higher doses and people with high vitamin k diets also need higher doses.

Not concerned.: Target range for INR is generally 2-3.  3.1 is barely out of range. It very slightly raises your risk of bleeding over your risk when you are at 3.0. Depending upon your doc this level may or may not produce a change in your dosing.

Factor VII , vit K: If pt. Is not on warfarin ( coumadi)...Lack of vitamin k, factor vii deficiency. If pt. Is on warfarin ...Steroids may increase it, also most antibiotics and a long list of meds that compete against the transporting protein. Check with hematologists.

Probably not ideal: Generally we like to see someone's inr be between 2 and 3 if you have a blood clot, although sometimes we make exceptions if you are taking Coumadin (warfarin) for other reasons.  Talk it over with your doctor.

Yes: Coumadin (warfarin) is a drug which inhibits the ability of your blood to clot properly.  If the inr gets above therapeutic ranges it increases the chances that you may have spontaneous bleeding.  This should be explained to you carefully by the doctor who prescribed this.  Go and see them asap.
