In what sense?: You've asked a very broad question here, and i'm not sure what you're getting at. If a patient were depressed, suicidal, and requested to be put to death, doctors would likely refuse these wishes.  They would see that the patient is ill and needs help.  Doctors are certainly known to listen to patients when going over treatment options for cancer, etc.  If i'm missing your point, please re-ask.

Many ways: We're all patients at one time or another, aren't we?  I'd be looking for a doctor with whom i can communicate, who's up to date in his/her specialty, who has time for me as a patient, and who's not afraid to say "i don't know."  i would ask my friends and colleagues for suggestions, and would probably also check internet reviews although those can be misleading. Practice web sites can tell a lot.

Extremely rare: Fortunately this is extremely rare.  But we sometimes see this with trauma patients, (gunshots) who present in critical condition.  We do exactly what we would do for non-surgical patients.-- we start cpr, and the anesthesiologist gives emergency drugs , blood or fluids needed.  We close the patient as soon as safe to do so with the intention of bringing back to or once stable (24 hours?).

Depends: There are alternative antibiotics to penicillin-type antibiotics.  The choice depends on the infection being treated.  There is testing available for penicillin allergy.  Those with penicillin allergy should see a board certified allergist for further evaluation.

Depends: In the emergency department it depends upon what medical problem you have.  If you need stitches, I would be spending the most time with you to finished closing your wound.  With a cough / cold / pneumonia the nurse would spend great deal of time giving medicines, measuring vital signs, and re-evaluation of your response.

Usually lidocaine: Lidocaine is commonly used because it is relatively safe, inexpensive, and most doctors have experience with it . It can be used in for suturing lacerations, dental procedures, and could be used topically (applied to the skin). The major shortcoming is that the effect doesn't last for a long time. There are other local anesthetics, including bupivacaine qnd tetracaine. Cocaine used to be used!
