: "Smoking marijuana" may not be detectable if stopped "a month prior to surgery", but it is not a contraindication to breast reduction surgery, and being forthright with your physician is always best, as other concerns about drug usage may alter the timing of the planned surgery.

Seriously, YES: Besides the obvious answer that long term marijuana use can be harmful, more importantly you are going to have general anesthesia.  General anesthesia has profound effects on the body and all the anesthesiologists i know would cancel your surgery if you were still smoking "weed" as would i.  Adding another dangerous drug to the mix of medications given during anesthesia is just plain silly.

: Heavy cannabis ingestion is correlated to less effective anesthesia. That said, many people have their wisdom teeth extracted under local anesthesia, so "waking up" is not a terrifying thing, it is something that can easily be reversed with more sedatives and anesthetics. At 17 your bigger concern should be your cannabis intake. Cannabis can interfere with mental development in puberty.

Smoking : Smoking causes vasoconstriction or tightening of the small blood vessels in the body. This leads to decreased blood flow and increases the risk of poor healing, infections, and in some instances loss of tissue. Patients who smoke before and after surgery are at greatest risk. Patients who stop smoking for at least 4-6 weeks prior to surgery may reduce some of the negative effects of a smoking history but will still be at higher risk than non-smokers.

Why: Is this grand dads worst habit?  Does it affect his relationship with you, or others, or the world?  Does it help him in some way?  He will only stop if he wants to, if it loses its favorable effect on him, or if he cannot get a supply.  You have probably already expressed your feelings and I am guessing that he has ignored you.

Cannabis: Tell him specifically (in a non-aggressive manner) what your concerns are.  You can encourage him to consider ma (marijuanna anonymous).  But you can't make him go. You can care for yourself by attending a 12 step program like al anon. If he is not willing to stop his cannabis abuse and you find it intolerable - ending the relationship is certainly an option.
