This may happen...: I assumed you mean using a 3-way indwelling catheter. If so, connecting the inflowing irrigation fluid to the large pore will soon develop severe bladder distention and make the patient go through roof. Normally, the irrigating fluid is supposed to be connected to the smaller pore. Such professional oddity has rarely happened so it's worthwhile to pay attention to the details of the procedures.

Separate locations: You may not actually see the structures easily but the pee hole is physically separate from your vaginal opening.It is located forward and above it.There would be no reason the urinary catheter would come in contact with your hymen.

Yes, it can be ...: Foley's catheter is the one with retaining balloon to keep it safely inside the bladder/ silicon is the material used to produce catheter, which is known to be less irritated to the urethra, but not much different for most people.
